A deep filling sits close to the nerve, which changes how it is placed. We line the base to insulate the pulp underneath, and the tooth can stay sensitive to cold for several weeks afterwards while it settles down. Sometimes it does not settle, and root canal treatment follows.
An abscess is an infection collecting at the root of a tooth, and it needs the source dealt with rather than antibiotics alone. That means either root canal treatment or removing the tooth. Antibiotics will settle the swelling for a while, but the problem returns once the course finishes.
Amalgam fillings that are sound, sealed, and not cracked are usually left alone.
